What can you expect when you make contact with Synergy?

Relatives Support

You can expect that our staff will treat you with respect at all times, in accordance with our equal opportunities policy.
We do not operate a drop-in service and ask that you self-refer by telephone if you are not being referred by a health professional or similar. 
We will ask you your name, address and few other personal details.   You will not be expected to give detailed personal information of a sensitive nature over the telephone.

 
 
Confidentiality and Information Sharing

Information is treated with the strictest confidence and nothing is shared or passed to third parties without your prior consent and approval. Agencies such as ourselves do have a legal duty however, to pass on information with or without your consent in the following
circumstances:
    Where there is concern that a child is at risk of harm
    Where there is a concern that you or other people may be at risk of harm
    The agency has been instructed to do so by a court
We share some information about you with the NDTMS if you give us your consent.  This is the database used to collect information on drug and alcohol treatment provision; It is managed nationally by Public Health England (PHE), an executive agency of the Department of Health.

 
 
Care Plans

Following an assessment, a Care Plan will be drawn up based on your individual needs.  This could include one or several different services within Synergy.
